Connections Housing Program Case Manager
Middleton Outreach Ministry is hiring a Case Manager for a new housing program, Connections, who will provide services from our main office, as well as conduct home visits, to assist people in finding and maintaining housing. The Connections Housing Program Case Manager is responsible for assisting participants who are in doubled up living situations to overcome barriers to stable housing by providing housing search assistance, case management, advocacy services, case planning, and referrals. The Connections Housing Program Case Manager will maintain an active caseload of up to 28 households (individuals or families) and will determine the length of time participants will be involved with the program (up to one year) after completing an intake assessment. The Connections Housing Program Case Manager will provide culturally inclusive, trauma-informed services and will incorporate best practice techniques into the program. Spanish-speaking ability preferred. Salaried position, plus vacation, sick-time, and other benefits. Some evening work may be required to best meet the needs of the people we serve.
